MCHD currently provides immunizations to children and adults and gladly accepts most insurance. All clients must provide a medical/insurance card: Some fees may apply. New clients must provide current immunization record.Immunization Clinic Hours

Third Tuesday of each month – Hours are extended until 6:00 pm
What to Bring for Your Immunization Visit:
- The patient’s shot record
- Photo ID and insurance card
- Registration and Consent Form
- Privacy Practice Acknowledgement
- and Preauthorization for Care Form if your child will be brought to his/her appointment by an adult other than the child’s parent or legal guardian.

How Much Will My Immunizations Cost:
Many healthcare plans cover recommended childhood immunizations as a preventive service with no out of pocket cost for the patient, but you should check with your plan to be sure what is covered for you.
Healthcare plans cover some adult immunizations as preventive services, but not all. Medicare Part B only covers flu and pneumonia vaccines. Adults are strongly encouraged to contact their healthcare plan to be sure what is covered for them.
If you have in-network healthcare coverage, you will be charged any required copay or deductible at the time of service. The rest of your visit will be billed to insurance. If any of your services are not covered by your insurance, you will be responsible for those charges.
Children with no insurance, insurance that does not cover immunizations, American Indians, and Alaskan Natives may be eligible for the Vaccine for Children (VFC) Program. No VFC child will be turned away for an inability to pay.
Patients with out-of-network insurance will be responsible for their charges. They will receive a receipt and charge ticket to turn into their insurance for reimbursement. Self-pay patients may be eligible for a sliding fee scale discount or a cash pay discount. Call to ask about your charges with applicable discounts.
