Obtaining Birth or Death Records

All Vital Statistics offices in Ohio are now able to issue certified birth certificates for anyone born in the State of Ohio.

The Morrow County Health Department maintains birth records occurring in Ohio beginning in 1908 and death records occurring in Morrow County beginning in 1908 to present. If the death occurred outside of Morrow County, the record may be obtained from the Ohio Department of Health – Vital Statistics Office or from the county in which the event occurred.

Certified Birth Certificate: $26.00
Certified Death Certificate: $26.00
Burial Permit: $3.00

Due to changes in State law, we can no longer provide uncertified copies of birth or death records.

Certified copies of birth and death records may be obtained in the following manner:

Effective October 15, 2015 pursuant to Ohio Revised Code 3705.23, social security numbers will be redacted from certified death certificate copies unless acceptable proof of relationship is shown and the “Proof Of Relationship Verification Addendum” is completed.

Mail-In Service

Please allow processing time of at least three weeks. Complete the APPLICATION for a Vital Record. Mail application form with a self-addressed stamped envelope and payment by check or money order made payable to Morrow County Health Department:

Walk-in Service

Come to the Community Services Building (619 West Marion Road – Entrance B) to apply for a birth or death certificate. Complete the application form and pay the fee. We accept cash, and check ONLY no credit or debit cards. In most cases, orders are processed at the time of request.

The Morrow County Health Department will not be liable for replacing certificates lost in the regular U.S. mail. If you want a more secure mailing method, you can chose to have your certificate sent by Certified Mail. This will be an additional cost. You will need to add $6.67 to your certificate request fee.

Note:  When choosing a government agency, please select either Cincinnati Vital Records or Franklin County Bureau of Vital Records. These are the only two agencies that offer on-line services to order birth certificates.

OBTAINING ARCHIVED RECORDS

Pursuant to Ohio Administrative Code, Section 3701-5-11, death records older than fifty years and birth records older than one hundred and twenty-five years may be removed from the Ohio Department of Health, Office of Vital Statistics (ODH/VS), and sent to the Ohio Historical Society for archival.

ODH/VS will not certify copies of records that have been removed from the vital records vault and sent to the archives at the Ohio Historical Society. Plain paper copies may be purchased from the Ohio Historical Society. Customers may obtain certified copies by contacting the local health district in which the vital event took place (local health districts are not subject to the same retention schedule as ODH/VS). The Ohio Historical Society houses Ohio death records from December 20, 1908 through December 31, 1963. A limited internet index of death certificates is available at www.ohiohistory.org/dindex.

Birth and death records from 1867 through December 19, 1908, were filed through the county probate courts. There is no statewide index to birth and death records from 1867 through December 19, 1908.
